Millennia Prepaid Card Documentation

Application form along with a photograph of the cardholder and any one of the listed OVD documents is required to apply for HDFC Bank Millennia Prepaid Card:

Officially Valid Document (OVD)

• Passport
• Driving licence
• Voter's Identity Card issued by the Election Commission of India
• Job card issued by NREGA duly signed by an officer of the State Government
• Letter issued by National Population Register containing details of the name and address
• Aadhaar Card (In case Aadhaar Card is provided by cardholder voluntarily, Aadhaar consent form is mandatory)
